Influencer Application Form Template

A good influencer gifting application form asks for the creator's name, email, handle, the product and variant they want, and a full shipping address, plus one or two screening questions. Build yours below, then copy it as a Google Form or run it on Shopify.

Tick the fields you want, set which are required, and the preview updates as you go. Nothing is sent anywhere, and the setup is saved in the page link so you can share it.

The 12 fields every gifting form needs

This is our recommended minimum, not a standard: the fields you need to approve a creator and ship a gift without a single follow-up email. Drop any that do not apply to your product, such as the variant for a one-size item.

FieldRequired by defaultWhy it is on the form
Full nameYesThe name on the parcel. Couriers and customs forms want a real name, not only a handle.
Email addressYesWhere you send tracking updates and follow-up. Without it you cannot reach the creator after they apply.
Phone numberNoSome couriers need a phone number to deliver. Make it optional unless your carrier requires it.
Main platformYesTells you where to check their content, and which deliverable to expect.
Your handle or profile linkYesThe one field you need to vet the creator before you ship anything.
Which product would you like?YesLets the creator pick something they would actually use, from a list you control.
Size, shade or flavourYesAvoids a back-and-forth email to find out the size. Skip it if your product has no variants.
Street addressYesWhere the gift is shipped. Ask for apartment or unit in the same line.
CityYesPart of the shipping address.
State / regionNoRequired for some countries and not others, so it is optional by default.
Postcode / ZIPYesCourier labels fail without it. A wrong postcode is the most common reason a gift is returned.
CountryYesDecides the carrier, the cost and whether customs paperwork is involved.

Screening questions that filter out freebie hunters

Screening questions do not make a form fraud-proof. They make a lazy application easy to spot. Keep two or three and make them optional if you would rather not lose genuine creators. Then check the account itself with the engagement rate calculator or the fake follower checker before you ship.

QuestionWhat the answer tells you
Link to a recent post you are proud ofShows the style of their content and proves the account is active.
Approximate follower countSelf-reported, so check it against the profile. Useful for sorting applications into tiers.
Why this product, in a sentence or two?A copy-pasted answer is a quick signal. A specific one tells you the creator looked at your product.
What would you make with it?Sets expectations about the deliverable before you ship.
Where is most of your audience based?A UK-based audience will not help a store that only ships to the US.
Have you posted about our brand before?Existing fans are often your best applicants, and you will want to know who has already received something.
Brands you have worked with recentlyA quick read on the categories they normally cover.
Hidden spam-trap fieldA field humans never see and bots often fill in. Needs a custom form: Google Forms cannot hide a question.

Example forms by niche

Each preset starts from the 12 core fields and adds the extras that tend to matter for that kind of product. They are starting points, so use the buttons in the builder above and then adjust.

Running it on Shopify

A Google Form works for a first batch. The cost shows up afterwards: every approved applicant has to be typed into Shopify as an order before anything ships. Seed is built around that step. You share one branded link, the creator picks a product and variant from your store and types their address, and a real $0 draft order appears in your Shopify admin.

  1. Install Seed from the Shopify App Store.

Connect your store so your products are available to pick from.

  2. Share your branded link.

Use it in DMs, emails and your bio instead of a Google Form.

  3. Creators apply.

They choose a product and variant and enter their address.

  4. The order is waiting for you.

A $0 draft order is in your Shopify admin, ready to fulfil.

Frequently asked questions

What should an influencer application form include?

At minimum: name, email, social handle, the product and variant they want, and a full shipping address. Add one or two screening questions, such as a link to a recent post and why they like the product, plus a disclosure checkbox. Keep it short, because every extra question is another chance for a creator to give up.

How do I add an influencer form to Shopify?

You can embed a Google Form or a third-party form on a Shopify page, but you then copy each application into your store by hand. Seed is a Shopify app that skips that step: the creator picks a product and variant, enters their address, and a $0 draft order appears in your Shopify admin.

How do I stop fake influencer applications?

No form stops every freebie hunter, but you can raise the bar. Ask for a link to a recent post, check the account before you ship, add a hidden spam-trap field to catch bots, and approve applications by hand. Limiting the choice to products you can afford to give away also caps the downside.

Can influencers pick their own gifted product?

Yes. Add a product dropdown and a variant field such as size, shade or flavour, and list only items you are happy to give away. In Seed, the creator picks from your Shopify products and the chosen variant goes straight into the draft order, so nobody has to email back and forth about sizes.

Methodology and sources

The builder is a deterministic template: your selections are turned into a field list, a preview, a CSV and a Google Apps Script by plain code in your browser. There is no AI step and nothing is uploaded. The field list, the screening questions and the niche presets are Seed's own recommendations, not an industry standard and not measured data. Treat them as a starting point.

The Google Forms script uses the Apps Script methods documented in the Google references above. Google Forms cannot hide a question, so the spam-trap field is left out of the script and needs a custom form.

The FTC states that free products or gifts from a brand are a material connection a creator should disclose. Rules differ by country, and none of this is legal advice. Ask your own adviser what your market requires.

Written by the Seed team. Seed is a Shopify app that lets DTC brands share one branded gifting link with creators and turns each application into a real $0 draft order. This page is deterministic: it runs in your browser, sends nothing anywhere, and the starting values are editable assumptions, not Seed data.
