# Campaign editor: Form fields

The Form fields tab, which contact and address fields creators fill in, shipping countries, parcel lockers, and custom questions.

The **Form fields** tab controls the second page of the gift form, the page where a creator enters their contact and shipping details.

## Contact & address fields

One row per field, each with a **Show** and a **Required** checkbox.

**1. First Name**

Always shown and required, needed to create the order.

Default: Shown, required

**2. Last Name**

Always shown and required, needed to create the order.

Default: Shown, required

**3. Email**

Always shown and required, needed to create the order.

Default: Shown, required

**4. Phone**

Can be hidden or made required.

Default: Shown, optional

**5. Instagram Handle**

Can be hidden or made required.

Default: Shown, optional

**6. TikTok Handle**

Off by default. Turn on to collect it.

Default: Hidden

**7. YouTube Channel**

Off by default. Turn on to collect it.

Default: Hidden

**8. Street Address**

Always shown and required, needed to create the order.

Default: Shown, required

**9. Apt / Suite / Unit**

Can be hidden or made required.

Default: Shown, optional

**10. City**

Always shown and required, needed to create the order.

Default: Shown, required

**11. State / Province**

Can be hidden or made required.

Default: Shown, optional

**12. ZIP / Postal Code**

Can be hidden or made required.

Default: Shown, optional

**13. Country**

Always shown and required, needed to create the order.

Default: Shown, required

> **Tip:** Fields marked "Needed to create the order" can't be hidden or made optional. Shopify can't create a draft order without them.

## Shipping countries

**14. Ships to …**

Your store's own country, resolved automatically from your Shopify billing address the first time a gift form loads. With just one country, the gift form shows no country field at all.

**15. Add a country**

Adds another country you're willing to ship to. Once you add at least one, the gift form shows a country dropdown listing your store's country plus every one you've added here.

## Parcel lockers

Shown only when your shipping countries include one that supports parcel lockers (for example Estonia). Creators shipping to one of those countries can pick a parcel locker instead of typing a street address.

**16. Carrier checkboxes**

For Estonia specifically, one checkbox per supported carrier, pick which carriers' lockers creators can choose from. Other locker-supported countries use a single network with no carrier choice.

## Custom question, second consent, marketing opt-in

These render below the fields above when the tab's full options are shown.

**17. Question**

One optional custom question you can add to the form, e.g. "What's your pet's name?" Leave blank to leave it off.

Default: Off the form (blank)

**18. Required**

Only enabled once the question above has text. Makes the custom question mandatory.

Default: Off

**19. Second consent. Checkbox text**

An optional second consent checkbox, separate from your Terms & conditions on the Review tab, e.g. "I agree the brand may use my content in paid ads."

Default: Off the form (blank)

**20. Second consent. Required**

Only enabled once the checkbox text above is filled in.

Default: Off

**21. Marketing emails opt-in**

An optional marketing consent checkbox, e.g. "Keep me posted about new drops and offers." Checking it on the form subscribes the creator to marketing emails in Shopify.

Default: Off the form (blank)
